3) QR Code of COOV App

4) Health declaration Form (HDF)

- Completion 72 hours prior to arrival at Guam (https://dphass.guam.gov/)

- The form will be distributed during flight to those who has not declared by web-site

5) Electronic custom declaration (EDF)

- Completion 72 hours prior to arrival at Guam (www.cqa.guam.gov)

- EDF may be proceed by KIOSK on arrival for those who has not declared by web-site
